Chapter 1 — What is CGHS?

Central Government Health Scheme (CGHS) is the comprehensive healthcare scheme of the Government of India for eligible Central Government employees, pensioners and their dependent family members.

Unlike ordinary medical insurance, CGHS provides healthcare through Wellness Centres, Government Hospitals, AIIMS, Institutions of National Importance and empanelled private hospitals. It includes OPD consultation, medicines, investigations, specialist treatment, hospitalization and medical reimbursement.

Chapter 2 — Family & Dependency Rules

CGHS covers the primary beneficiary, spouse and eligible dependent family members. The spouse is always eligible, whereas most other relatives must satisfy the prescribed dependency conditions.

Important: A Government employee can normally include either parents or parents-in-law as dependents—not both simultaneously.

Who is treated as dependent?

For parents and other relatives, dependency depends upon the income limit prescribed under CGHS. The spouse is exempt from this condition and continues to remain eligible even if receiving family pension.


Chapter 3 — CGHS Card: Application, Documents & Timeline

The procedure differs slightly for serving employees and pensioners. Serving employees apply through their department, whereas pensioners apply directly to the CGHS Additional Director after making the prescribed contribution.

Chapter 4 — CGHS Contribution & Ward Entitlement

Serving employees contribute monthly through salary, while pensioners may choose yearly contribution or a one-time contribution for lifetime CGHS validity. The applicable contribution depends upon the Pay Matrix Level under the 7th CPC.

Chapter 5 — Referral Rules & Direct Consultation (2024)

This is the most important chapter because most CGHS beneficiaries are confused about when they can directly visit a doctor and when a referral from a CGHS Wellness Centre is compulsory.

Golden Rule: Government Hospitals generally do not require CGHS referral. Planned consultation in empanelled private hospitals usually requires referral unless specifically exempted.

How long is a referral valid?

  • Specialist referral: Valid for 3 months.
  • Within these 3 months, consultation with up to 3 specialists is permissible if recommended.
  • A maximum of 6 specialist consultations is allowed during the validity period.
  • Routine listed investigations do not require fresh endorsement once advised by the specialist.

Chapter 6 — Emergency Treatment & Cashless Hospitalisation

CGHS beneficiaries are entitled to immediate medical treatment during a genuine emergency. The 2019 Ministry of Health Office Memorandum makes it clear that emergency treatment cannot be denied merely because a referral is not available.

Emergency means first treatment, paperwork later.
If delay is likely to endanger life, limb or essential organ function, the patient should immediately report to the nearest suitable hospital.

What should you do during an emergency?

1. Who is eligible for CGHS?

CGHS beneficiaries are entitled to prescribed medicines and diagnostic investigations according to the approved treatment protocol. Medicines may either be available at the Wellness Centre or supplied through the authorised indented medicine system.
